Examples of Surveys and Questions PDF

How to Make the Survey:

  1. Make your survey using Google Forms or Survey Monkey or something similar. Send to family, friends, faculty, classes, acquaintances. Post on social media channels, Discord, Reddit, and more. Try to especially get it in the hands of your demographic.
  2. Watch this video on how to create Google Form.
    • Note: To make sure that anyone can access your survey outside of WWU. Do the following:
      1. Go to Settings
      2. Go into Responses
      3. Toggle OFF the “restrict to users in Western Washington University and its trusted organizations.”

How to structure a survey

  1. Introduce them to the survey and explain briefly what it is for. Thank them for their time!
  2. Include questions to gather relevant demographic information at the beginning.
  3. Ask easier to answer questions at the beginning.
  4. Get into more complex questions in the middle.
  5. End with more general easier questions.
  6. Allow them the opportunity to add feedback or additional comments.

Tips

  • Ask around 8-10 questions or test the survey out and just make sure it doesn’t take more than 7-8 minutes.
  • Include both closed and open-ended questions. This will allow you to get both easy to assess quantifiable data and more in-depth qualitative data for insights.
  • Ask questions that will help validate your hypotheses and discover pain points and opportunities.
  • Figure out some main goals for the survey or ask yourself what you want to learn from it. What information are you seeking that can help you improve this product?
  • If you are struggling to come up with questions about people’s experience with your product or similar products, you can always refer back to using the Who, What, When, Where, Why, and How questioning method to assist you.
  • Avoid leading questions, double-negatives, double-barreled, unbalanced scales, and bias.

How to summarize results

  • Group open-ended answers and look for repeating patterns.
  • Use charts, graphs, and tables to show some of the closed-question responses. (Google Forms provides these in pie and bar charts)
  • Provide written summary of key takeaways.
  • Look for things that give new insights, validate your hypotheses, or answer the goals you set for the survey.
  • Include how many respondents you had and the collected demographics from this sample.
  • You can also include direct quotes.
